Illness anxiety involves checking too, but often less ritualized.
- The company it keeps. Health OCD frequently travels with other OCD themes, past or present โ contamination, harm, sensorimotor fixations. If illness fears are one act in a longer OCD story, that's telling.

The Trap Both Share

Reassurance is the debt that compounds.

The Treatment That Works for Both
The evidence points the same direction for health OCD and illness anxiety: cognitive-behavioral therapy built around exposure and response prevention. In practice that means:
- deliberately facing illness uncertainty (reading about the feared disease, writing "I might get sick someday and can't be certain") without neutralizing it,
- cutting the checking โ no scanning, no Googling, no reassurance rounds,
- often, scheduled reasonable healthcare (one annual physical, symptoms lasting >2 weeks get one visit) to replace the binge-avoid cycle with rules made in a calm moment,
- and tolerating the anxiety spike until your brain learns that unanswered doubt is survivable.
SSRIs help many people with either presentation, particularly when depression rides along.
One practical warning: this is a presentation where generalist therapy commonly goes wrong. Supportive counseling that explores your fears โ or worse, helps you get reassurance more efficiently โ feeds the mechanism. You want someone who treats health anxiety and OCD with exposure methods, on purpose, regularly.
