๐ง OCD vs. ADHD: Why They Get Confused (and Why It Matters for Treatment)
You can't finish tasks. You're mentally exhausted by noon. You read the same paragraph four times. Depending on which clinician you see โ and which TikToks you've watched โ that description gets you an ADHD workup or an OCD screening. Sometimes the wrong one.

Here's the direct answer: OCD and ADHD can look alike on the surface โ poor concentration, unfinished tasks, restlessness โ but they run on opposite engines. OCD is a disorder of over-control driven by doubt and anxiety; ADHD is a disorder of under-control driven by executive dysfunction. They also co-occur in a meaningful minority of people, and the distinction changes treatment significantly.
Same Symptoms, Opposite Engines
Consider "I can't focus":
- In ADHD, attention slides off the task because the brain's executive system under-engages. The distraction is anywhere and everywhere โ novelty wins.
- In OCD, attention is being actively consumed by something specific: an intrusive thought, a mental review, an internal argument. The person isn't under-focused; they're intensely focused on the wrong thing, involuntarily.
Or "I take forever to finish things":
- In ADHD, tasks stall from procrastination, boredom, or losing the thread.
- In OCD, tasks stall because they can't be finished wrong โ rereading the email eleven times, redoing the calculation, waiting until it feels right.
Or restlessness: ADHD fidgets because stillness is under-stimulating. OCD fidgets because sitting still with an unanswered obsession is unbearable.
The Questions That Separate Them
A clinician who knows both conditions is listening for the why underneath the behavior:
- What happens if you're interrupted mid-behavior? Interrupted ADHD wandering is painless. Interrupting an OCD ritual produces spiking anxiety or a need to start over.
- Is the repetition serving a fear? Checking a lock ten times to prevent catastrophe is OCD. Checking for your keys because you genuinely lose them weekly is ADHD.
- Was it there before the anxiety? ADHD traits typically trace back to early childhood across settings. OCD more often has an onset โ a period when the thoughts arrived.
- Does the mind feel empty or crowded? ADHD distraction often feels scattered and blank. OCD distraction feels loud, sticky, and specific.
When You Have Both
This isn't either/or. Studies suggest a meaningful overlap โ a significant minority of people with OCD also meet criteria for ADHD, and the combination is especially common in childhood-onset cases. Having both is its own clinical puzzle:
- Untreated ADHD makes ERP harder โ showing up, planning exposures, and tolerating structured practice all draw on executive function.
- Untreated OCD makes ADHD treatment misleading โ no stimulant fixes attention that's being eaten by obsessions.
There's also a genuine clinical wrinkle worth knowing: in some people with OCD, [stimulant medication can sharpen focus on the obsessions themselves](https://iocdf.org/expert-opinions/expert-opinion-ocd-and-adhd-dual-diagnosis-misdiagnosis-and-the-cognitive-cost-of-obsessions/), making symptoms feel worse. That's not a reason to avoid treating real ADHD โ it's a reason the diagnosis needs to be right before the prescription is written.
Why the Label Changes Everything
Get this call wrong and both roads fail. ADHD treatment for what's actually OCD leaves the fear engine untouched. Talk therapy or OCD medication for what's actually ADHD leaves the executive deficits untouched. And "we tried things and nothing worked" becomes its own discouraging story that keeps people from trying again.
The fix is unglamorous: a proper differential assessment by someone who treats both conditions routinely, using structured tools rather than vibes. If OCD is any part of your picture, that means a clinician who can recognize mental compulsions and knows what ERP is โ which, as we've written before, is rarer than it should be. We also compared OCD against autism and OCPD in an earlier diagnosis guide if your question spans more than these two.

Can you have OCD and ADHD at the same time? Yes. They co-occur in a meaningful minority of cases, particularly with childhood-onset OCD. Each condition can mask or complicate the other, so assessment by someone who knows both matters.
How can I tell if my focus problems are OCD or ADHD? Ask what's stealing the focus. If your attention is being consumed by specific repetitive thoughts, doubts, or mental rituals, that points toward OCD. If it drifts toward anything novel and always has, that points toward ADHD.
Do stimulants make OCD worse? For some people, stimulants intensify obsessive focus; for others with genuine comorbid ADHD, treating the ADHD helps overall functioning. This is exactly why the diagnosis needs to precede the prescription.
Is ERP still possible if I have ADHD? Yes โ it may just need more structure: shorter exposures, written plans, reminders, and a therapist who adapts pacing. Sometimes treating ADHD first makes ERP dramatically more doable.
